Thank you for choosing to share your child’s story with the Take Part community. Completing this form will instantly create a draft Warrior Profile—your child’s own page on our site—so friends, family, and supporters can follow the journey and lend a hand.

Only three essentials are required: Name, Diagnosis (or “Undiagnosed”), and a one-sentence bio. Everything else—photos, longer story, Google-Drive links—can be added or edited later at your convenience.

How it works (30 seconds):

1. Fill in the essentials and click Submit.
2. You’ll receive a secure login email. From your personal dashboard you can upload photos, add Drive links for medical records, or update details anytime.
3. Your profile will instantly be created. A default “Leon the Lion” image will stand in until you upload your own.

Data & privacy: Your information is stored securely, never sold, and used only to power your Warrior page and related Take Part programs. You may request edits or removal at any time.
